What exactly does Zero Balance spend?
Short answer
It spends the leftover sitting on your Apple ID - the same balance that can keep you from switching App Store region. You enter the amount, the app lines up a few small App Store purchases to match it, and you see the total before you confirm.

Zero Balance only ever touches the store credit already sitting on your Apple ID - the leftover that is often the reason the App Store will not let you change country or region. It does not pull from your card unless your balance runs out mid-plan, because Apple always spends the Apple ID balance first. You enter how much is left, the app works out a small set of in-app purchases that adds up to roughly that amount, and the plan screen shows you the total and any overage. Nothing is bought until you approve each item through Apple's own prompt, so you stay in control the whole way.
